DSU Registration Form 2013-2014 Complete this form when registering for DSU After School Programs including Alden DSU Overtime, Alden DSU One Hour, DSU CEO Program for Middle School, DSU Access Pass for Middle School, 10 – Visit Punch Cards for Middle School.
DSU Camp Form 2012-2013- For Early Release Days, Professional Development Days, and Summer Camp. One-time form to be filled out when registering for any of these special event camps and will be kept on file for the school year. If child is registered in a DSU After School Program, no camp form is required.
Bus Form 2012-2013 – A bus transportation form must be filled out and submitted to the Alden office when an Alden student plans to ride the DMS/DHS bus with an older sibling.
Alden Dismissal Form 2012-2013 – Please use an Alden Dismissal Form to inform Alden School and your child’s teacher of your plans to attend DSU. Can be used for our everyday after school program or for camp days.
DSU_Facility_Use_Form – To be filled out when renting DSU meeting, party, or event space.
